WHAT IS FETAL MEDICINE
Fetal medicine is an upcoming branch of Obstetrics and Gynaecology. As fetal medicine specialists, we are specialized in performing
-
Antenatal Ultrasound examination of the unborn baby i.e. fetus.
-
Monitor the proper growth of fetus according to the gestation of the pregnancy,
-
Look for the normal development of the fetus, detect abnormalities if any
-
Guide and provide counseling to the couple undergoing the necessary tests while taking any decisions in case of high risk pregnancies.
1
Fetal Anomaly Scanning
An anomaly scan is done to find out if there is any abnormality in the developing baby and to decide on it’s cure or precautions throughout the pregnancy and also during child birth. It is one of the most important pregnancy scan which provides detailed look at how your baby is growing, position of placenta, the amniotic fluid and also helps us know how are the blood flows going to the fetus. We can also predict if there is a possibility of developing high blood pressure in pregnancy i.e. pregnancy induced hypertension in the later months of pregnancy
2
Fetal Echocardiography
A fetal echo or fetal echocardiography is an ultrasound test performed often as early as 18-24 weeks of pregnancy. This test is to evaluate the structure and function of your unborn baby’s heart. If any previous ultrasound tests raised a suspicion of an abnormality in the fetal heart, detailed fetal echocardiography is recommended. If mother is a known diabetic , or has taken any medications or if there is a family history of cardiac defect then with detailed echo a prenatal diagnosis of fetal congenital heart disease (CHD), shows a significant effect on prenatal and postnatal management of childbirth, care and it’s outcomes.
Echocardiography or Echo assesses the heart's structures and function of any fetus by placing a small ultrasound probe on the mother’s abdomen. The ultrasonic sound waves at a frequency too high to be heard move through the mother's and baby's skin and other body tissues, where the waves "echo" off of the heart structures. A Fetal Medicine expert is trained to have in depth look and find any defect or abnormalities.

Fetal Growth and Doppler Study
Routine antenatal care is to rule out or clinically deal with the risk factors associated with your fetus. With changing time and life challenges, high risk pregnancies are on high and routine check up or antenatal care becomes imperative in most of the pregnancies now a days. This ultrasound scan is usually carried at about 28 – 36 weeks of pregnancy and aims at determining the growth and health of the fetus.

A pathological decrease in the rate of fetal growth and movements defines Fetal growth Restriction. There maybe various causes of this fetal growth restrictions which need to be figured out by a Fetal Medicine Expert.

Fetal growth and Doppler study is particularly important for the pregnant women who have experienced problems with their pregnancy, pre-eclampsia, diabetes, stillbirth, and fetal growth deficiency.

Invasive Procedures
Now a days, genetic diseases are an indication for invasive procedures and their prenatal diagnosis becoming increasingly important. Dr. Priya Kumbhare (Deshpande) has performed various invasive procedures like:

-
Amniocentesis : An amniotic fluid test or AF is used to diagnose any Prenatal Chromosomal abnormalities in Fetus and Fetal infections. Amniocentesis is performed between 16 and 20 weeks of pregnancy. This test is primarily for those expecting mother’s who are at increased risk for genetic and chromosomal problems. . The most common reason to have an your Amniocentesis test (AF) is to determine whether a baby has chromosomal abnormality, such as Down’s Syndrome. In this test a small sample of the amniotic fluid around the baby is taken under ultrasound guidance and sent for testing to the laboratory.
-
Chorion Villous Biopsy: Chorionic villus sampling (CVS) or chorionic villus biopsy, is a test performed during first trimester of pregnancy i.e. before 13 weeks to detect chromosomal abnormality of genetic syndrome. CVS test is performed by taking a sample of the chorionic villi.(placental tissue). The genetic information in the tissue helps in finding out if an abnormal genetic condition is present, such as Down syndrome, Thalessemia or other genetic syndrome. This helps them to decide how to proceed with the pregnancy.
-
Fetal Reduction: Being pregnant with two or more than two fetuses has it’s own challenges. It’s more likely that all babies will not be healthy. A procedure to lower the number of fetuses and increase the chances of healthy pregnancy and survival is called selective fetal Reduction. The more babies a lady carries the more chances of having a miscarriage preterm delivery or stillbirth. Premature delivery can cause problems in your baby in lungs, heart, stomach, and brain. They could also face lifelong health issues like cerebral palsy, mental retardation, and vision and hearing loss.
